  • FERPA Release Form
    • The TRU-UE Information Consent Form is now available in the Personal Info section of SIS Student Self Service (https://sis.jhu.edu/sswf/).
    • In addition to the directory information that JHU provides to TRU-UE to the extent permitted by the Family Educational Rights and Privacy Act (FERPA), you may complete the TRU-UE Information Consent Form to elect to disclose supplemental information to the union for the purposes of representing you.
    • Additional information about this form can be found on the Office of the University Registrar’s FERPA Requests webpage.
    • Special Note for Students with Directory Information Restrictions:
      • Please note that the TRU-UE Information Consent form is not available to students who have restricted their directory information by following the process described on the Office of the University Registrar’s Directory Information webpage.
      • Students with directory information restrictions who wish to share education records with TRU-UE may either (a) remove the restrictions on the disclosure of their directory information; or (b) submit a Student Information Release Authorization form consenting to the disclosure of education records to the union by including the following information in the form:

        • Identify education information that may be released to the union
