The TRU-UE Information Consent Form is now available in the Personal Info section of SIS Student Self Service (https://sis.jhu.edu/sswf/).
In addition to the directory information that JHU provides to TRU-UE to the extent permitted by the Family Educational Rights and Privacy Act (FERPA), you may complete the TRU-UE Information Consent Form to elect to disclose supplemental information to the union for the purposes of representing you.

Special Note for Students with Directory Information Restrictions:
Please note that the TRU-UE Information Consent form is not available to students who have restricted their directory information by following the process described on the Office of the University Registrar’s Directory Information webpage.
Students with directory information restrictions who wish to share education records with TRU-UE may either (a) remove the restrictions on the disclosure of their directory information; or (b) submit a Student Information Release Authorization form consenting to the disclosure of education records to the union by including the following information in the form:
